The complex/team lead social worker is responsible for assessing and assisting in meeting psychosocial, resource, and financial patient needs for the most complex patients-(trauma, long length of stay, uninsured with complex medical needs...), throughout their stay regardless of what unit they are on, providing better continuity, increased customer service and more timely discharge. In collaboration with case managers, staff members, physicians, and other healthcare team members, the complex/team lead social worker provides evaluation of the appropriate level of care, and develops relationships with providers in the community to facility more timely patient transition to another level of care or resources to facilitate transition home. The complex/team lead social worker will be the first point of contact for the other social workers for routine questions on a day to day basis. They will provide ongoing education to the team to improve skill sets and knowledge of resources for complex patients. The complex/team lead social worker will cover for the social work supervisor in his/her absence and escalate issues as appropriate to the Director.
Master's Degree in Social Work with at least two years healthcare social work experience. LMSW required within 1 year of hire.
