The Care Coordinator's primary responsibilities are to provide outreach, support, and coordination of care for patients across the health care continuum. This individual is expected to collaborate with all clinical and non-clinical staff to assist patients with meeting their preventive and chronic health needs. The Care Coordinator will work cooperatively with practice personnel to best serve the needs of identified patient populations through the following activities:

- Promoting timely access to appropriate care

- Increase utilization of preventative care

- Follow-up with patients within a few days of an emergency room or hospital visit

- Work with patients over the phone to remind and review their plan of care and progress toward goals

- Establish and maintain relationships with identified service providers

- Increase continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals

- Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs.

- Assist with data collection and generation health data as deemed appropriate for the management of a specific population,

- Participate in Performance Improvement/Continuous Quality Improvement activities, as assigned

Minimum Qualifications:-
  • Associate's degree in a health related field or LPN or High school diploma or General Equivalency Diploma (GED) with Diploma as a Medical Assistant (MA) or completion of state-approved Certified Nursing Assistant (CNA) training program and

• 1-3 years of clinical care experience

• Ability to work collaboratively with physicians and healthcare administrators

• Ability to work independently under general direction

• Skilled in oral and written communication

• Proven organizational skills

• Resourcefulness in problem solving

• Ability to demonstrate excellent customer service and strong cultural competency

• Ability to adhere to strict confidentiality guidelines

Preferred Attributes/Characteristics:
  • Knowledgeable with HEDIS, MIPS, Meaningful Use, & other pay for quality initiatives.
  • Medical terminology
  • General knowledge of common chronic diseases, including but not limited to: Diabetes, Hypertension, Asthma, COPD, Chronic Kidney Disease, Cardiovascular disorders
  • Prior experience with care coordination/care management in a medical setting
  • Prior experience with managing competing priorities
  • Analytical skills necessary to analyze and interpret routine data.
