Position Summary:

Medication Reconciliation Facilitator is a support position utilized in the emergency department. R e sponsibilities are to collect, organize and document medication histories for patients under the direct supervision and responsibility of a nurse. The facilitator will assist with providing safe medication management of the patient by confirming the current (home) medication list at the time of admission to the emergency department and inputting information into the patient's medical record. Duties include obtaining a current list from the patient and/or patient's family (or other representative when appropriate) that include prescription medications, over the counter medications, herbal remedies, verify allergies and supplements. Additional information will include if medications are taken on a schedule, or as needed (PRN) basis, the medication dose, route of administration, and frequency. Verbal communication of the completion of the medication reconciliation process to the primary RN or via the electronic tracking board in accordance with the current EMR.

Minimum Qualifications:

- Graduate of an accredited school of nursing

- Current State of Georgia Registered Nurse License

- Current Healthcare Provider BLS

or

- Pharmacy Technician Certification or eligible for recertification

- Two (2) years of recent clinical experience as certified pharmacy technician

- Registered Technician with State of Georgia

- Current Healthcare Provider BLS

- High school graduate or the equivalent.

- Proficiency in computer data entry

- Proficiency in English Language
